DEADLINE for device request is Friday January 8th, at 4 p.m.

Information Regarding Lending a device to your child:

Please complete the form above if your child requires the use of a technology device for remote learning (January 4 to Jan 25, 2021).

Documentation will accompany the device:  UGDSB Home Use of Technology Agreement, which includes tracking information and cost of replacement of the device if not returned.

Please fill in ONE form per each child requiring a device. Incomplete information will delay processing of the requested device. DO NOT enter more than one student’s name on the form.

Details regarding the pick-up of devices MUST be scheduled with the Main Office staff.  Once you submit the form above, you will receive a phone call or email advising you when your device is ready for pick up at JFR.  On the date and time of your device pick-up, please enter the school at the front/main doors only, and pick-up the device on the table inside the main doors.

If your child does not have access to the internet, please note that this is a separate request and may take time to process the required access.  Please send such request to Mrs. Sanvido at [email protected], or contact her directly at 519-822-7090, ext. 306.
