Chromebook Requests for April 7, 2021 for Remote Learning

Dear Parents/Guardians:

The Upper Grand District School Board has been notified that Wellington-Dufferin-Guelph Public Health will be directing all UGDSB schools to shift to remote learning effective Wednesday, April 7th, 2021.

If you require a Chromebook for your child to participate in Remote Learning, please click on the link below to complete the form.

Once we have received your request for a device, we will be sending the devices home with students on April 6th, 2021. 

For parents/guardians whose children are absent from school on April 6th devices will be available for pick up on Wednesday, April 7th between 9:00 and 3:00. When coming to the school to pick up a device, you must complete a Covid self-assessment for yourself:

○  Be symptom-free of respiratory illness 

○  Have not been outside of Canada within the last 14 days from the date of entry to the school

○  Have not cared for or had known contact with an individual with confirmed COVID-19 or COVID-19 like symptoms (fever, new cough, difficulty breathing).

If you do not meet all of the conditions you MUST NOT come to the school and other arrangements must be made.
