Plans of Care for Students with Medical Conditions!
Dear Parents/Guardians,
I hope this message finds you well and you are enjoying your summer!
Please see the link below for any additional forms you may need around Plan of Care for your child/ren for the 2022-2023 school year.
Thank you very much for your support in making sure we have the most updated information for the plan of care for our students.
If your child has a Plan of Care in place and no changes are required, we need you to complete the Annual Renewal Form (518.6), however, if there are changes, we will need you to complete the appropriate form with all the details.
If you require a paper copy, kindly email me at [email protected] and let me know and I will arrange to have a copy ready for you to pick up or send home with your child/ren.
