If there are new or significant changes please complete the applicable forms: https://www.ugdsb.ca/board/policy/policy-518-students-with-life-threatening-medical-conditions/

Hard copies are available in the main office.

Please return your form on the the first day of school, Tuesday September 3rd.

If your student no longer requires a Plan of Care, please notify the office.

The 2019 Long Term Accommodation Plan (LTAP) Annual Review Report was presented at the Business Operations Committee meeting on March 19, 2019.

This purpose of this report is to review updated data and any other changes affecting accommodation. It is necessary for staff to monitor the relevance of the Board’s work plan priorities. The LTAP review report also provides the status of the 5-year work plan priorities identified for 2018-2022.

Smoke and Vape–Free School Environment

The Upper Grand DSB provides a smoke and vape-free environment for its students, staff and others while on school Board property and at school events.  Under our policy, all forms of tobacco, and any processed form of tobacco that may be smoked, inhaled or chewed is included. Additionally, all forms of e-cigarettes are also covered under this policy.
