Students with Life-Threatening Medical Conditions (Asthma, Diabetes, Epilepsy/Seizure Disorder, Anaphylaxis, etc.)
The UGDSB is committed to minimizing the risks for students who have asthma, diabetes, epilepsy/seizure disorder, life-threatening allergies and/or other serious health condition through the development and implementation of individualized Plans of Care. Please contact the office at 519-843-2665 if your child requires a plan of care.
Please note: Plan of Care information was sent home with those students who attended Westminster Woods last year. Please ensure plans are updated, signed and returned to school with required medications on the first day!
