Hello parents & guardians,

I hope you and your loved ones are doing well and enjoying the summer break!

Please find attached in the link below for Students with Life-Threatening Medical Conditions and the Annual Plan of Care Renewal form based on our current school year and those students who will need their Plan of Care in place for the upcoming 2023-2024 school year.

We are asking that you kindly complete the appropriate form and return it to me for the upcoming school year at your earliest convenience. Please complete the form fully including your child’s name, date of birth, Original Date of Plan of Care, Plan of care for, in effect for the 2023-2024 school year, sign and date.

If there are any changes required for the upcoming year, kindly reflect those changes in the appropriate form.
