Life Threatening Medical Conditions

Re: Plan of Care for Students with Serious or Potentially Life-Threatening Medical Conditions

The Upper Grand District School Board (UGDSB) is committed to ensuring a safe, healthy, and inclusive learning environment for all students. Students with serious or potentially life-threatening medical conditions — such as asthma, diabetes, anaphylaxis, epilepsy/seizure disorder, or other conditions that pose a serious health risk during school-based activities or while using school transportation — require an individualized Plan of Care.

Important: New Plan of Care Required for All Students in 2026–27

The board has updated its Plan of Care forms. As a result, every student with a Plan of Care — including students with an existing plan on file — must submit a new Plan of Care using the updated forms for the 2026–27 school year. The Annual Renewal option is not available this year.

To complete your child's Plan of Care:

  1. Complete the appropriate Plan of Care form, available at ugdsb.ca/o/ugdsb/page/policy-518-students-with-life-threatening-medical-conditions

  2. Obtain your health care provider's signature, as appropriate

  3. Schedule a meeting with the principal (or designate) to finalize the Plan of Care

Your role as a parent/guardian

  • Help your child understand their medical condition(s)

  • Encourage your child toward self-management and self-advocacy where appropriate

  • Inform the school principal of your child's medical condition(s) within the first 30 days of the school year, or as soon as possible, and help develop your child's Plan of Care

  • Review the Plan of Care and communicate any changes to the principal — at minimum annually, within the first 30 school days, and immediately if there are changes to your child's medical condition(s) or their ability to self-manage

  • Supply your child and/or the school with sufficient, clearly labelled quantities of medication and supplies as directed by your health care provider, and track expiration dates

  • Keep emergency contact information current

For full details, please refer to Policy 518 and its associated procedures, available on the board website.
