Immunization Clinic – December 5th & Vaccine Reporting to Public Health

Parents/guardians of students who have incomplete records on file with WDGPH were mailed out immunization notices.  The notices were mailed out for students who are overdue for mandatory vaccines and for students who are overdue for recommended vaccinations (HPV, HB & Varicella).

CHSS Vaccination Clinic Date is Tuesday, December 5, 2023.

Suspension Letters:  will be delivered to the schools February 6 & 7, 2024 for distribution to the students.

Reporting Deadline:  February 9, 2024, by 4:00 PM. Suspension Day is February 14, 2024.

It is a parent’s/guardian’s responsibility to report their child’s vaccinations to Public Health in order to attend a licensed childcare facility or school.

Online:  www.immunizewdg.ca  using Ontario Health Card #  (parents can also view current, overdue and upcoming immunizations here)
