Medical Plans of Care:

ODSS keeps a Plan of Care on file for all students with life-threatening medical conditions. There are specific forms for Anaphylaxis, Asthma, Diabetes, and Epilepsy, and a general form for other conditions.

If your child requires a Plan of Care, please call or email the office so we can send you the correct form. Plans must be in place as soon as possible to ensure your child’s safety.
