Immunization Vaccine Reporting to Public Health

It is a parent’s/guardian’s responsibility to report their child’s vaccinations to Public Health in order to attend a licensed childcare facility or school.  You can report your child’s vaccines:  Online:  www.immunizewdg.ca  using Ontario Health Card #  (parents can also view current, overdue and upcoming immunizations here)  Email: Send a copy of your record to [email protected]   Questions: Contact the Client and Community Support Line: 1-800-265-7293 ext. 7006 (Mon–Fri 8:30am-4:00pm)   For those who need help translating their records please contact Immigration services for the translation of immunization records into English.
