Important information regarding WHY Survey – a joint survey between UGDSB and WDG Public Health

GUELPH, Ontario – Wellington-Dufferin-Guelph Public Health has partnered with Upper Grand District School Board to conduct a survey in your child’s school. All students in Grades 4 through 12 will be asked to complete this survey as well as parents in elementary schools.
Students:
Completion of the survey is not required for your child’s schooling and it won’t affect their grades. Completion of the survey would provide us with valuable information and would be appreciated.
WDPH and UGDSB have partnered to create one survey to replace our school climate, mental health and youth health surveys. The new survey will provide the information we need to inform programs and supports for students. The results will also help identify opportunities to improve the well-being of children and youth in our community.
The Research Committee of the Upper Grand District School Board has given permission for this study to be carried out at your child’s school. All information collected will be strictly confidential and the students will not be identified individually. Your child’s name and any other information that could identify your child will not be collected in the survey. Your child’s participation is completely voluntary.
Secondary students will be asked to complete this survey during their English Class. Students who do not take English during first semester will have the opportunity to complete the survey in February.
The survey will be conducted at your child’s school November 4th – 15th.
Parents/Guardians:
Parents and guardians are being asked to complete a similar survey. Like the student survey, this survey will be confidential and the results will be used in the same manner as the students survey.
Parents will be asked to participate in this optional survey during the month of November. Computers will be available to parents in November during parent/teacher interview night at their child’s elementary school.
